Forms - To download, right click on the underlined form name that you want, then on the popup menu "Save Target As" and choose to where you want to download the form. Suggest you create a folder in documents call Auxiliary E-Forms. In order to enter information on the forms you will need Adobe Acrobat Approval or Professional. To record your activity use forms 7929 and 7930.
Effective Jan 1, 2008, the 7030 mission activity report form will have a number of major changes incorporated. Many of the mission activity codes have been dropped or revised. In addition the mission code descriptions shown on pages 3 through 8 of the 7030 have been changed and expanded so it is important to print these pages and have them accessible when filling out the new form.
